All benefits are subject to change, be eliminated or reduced at the Company's discretion. 20-Year Pension: The International Association of Fire Fighter Local I-60 negotiated and received in their 2003 signed contract a 20-year pension for field employees under their bargaining unit. Dental Plan: Our dental plan offers the benefit of using in-network or out of network providers for preventative care, general services and orthodontics. Vision: This program provides benefits for eye exams, eyeglasses and contacts. Life Insurance: Basic coverage is provided to employees, spouse and children. In addition, supplemental life insurance may be purchased for the employee, spouse and children. Flexible Spending Accounts: This benefit allows you to set money aside for expenses you incur for medical and dependent care. Your Flexible Spending account provides an income tax benefit. All deductions are taken on a pre-tax basis. Disability Insurance: Provides replacement of a portion of your income if you cannot work due to an illness or injury. Employee Assistance Program: A full range of Company paid counseling is provided for personal and work -related concerns. 401(k): You may contribute up to 15% of you pre taxed income. The Company may make a discretionary contribution up to 100% of the first 2%. Vacation: Southwest Ambulance recognizes the importance of paid vacation time for rest, recreation and personal activities. Vacation award is based on full-time years of service with the Company.
