During an emergency at your home, there are times when you may be unable to speak or think clearly. Let Southwest Ambulance's "Envelope for Life" speak for you.

1) Download and print the attached form and fill out the medical information requested for each resident of your home.
2) Put the information in an envelope and write: "Envelope for Life" on it.
3) Wipe dry the inside rightward wall of your refrigerator and tape the envelope to the side.
4) Download and print the attached small logo. Cut the logo as indicated, and then tape it to the outside top left corner of your refrigerator\'s door to alert emergency medical personnel that the envelope can be found inside.
5) Be sure to update the form regularly.
